The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.
NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.
We will be sending reminders to patients for some of the following services if you are due for them, Mammograms, Bone Density Tests, Colonoscopy, Pulmonary Function Tests, Diabetic Screening tests, Immunizations and other health maintenance services.

Please ask that ALL of your hospital, specialist, imaging or laboratory records are faxed to Dr. Yasin which is your PRIMARY CARE PHYSICIAN.

* It is necessary to Schedule a follow up appointment with your primary care physician within 2 weeks of your discharge from any ER, Urgent Care visit, Hospitalization, Rehab or Surgery. Make sure you bring all discharge paperwork including medications with you to your follow up appointment *
